The lead single, "Don't Be Sad", was co-written by The Smashing Pumpkins guitarist James Iha, who also provided guitar contributions and backing vocals. The album is known for its troubled history, having sat on a shelf for two years after the band lost their record deal. Several bootlegs of the album surfaced before the official release from Lost Highway Records.


The album was met with positive reviews from critics. AllMusic gave it four out of five stars, declaring it as an "undeniably striking and beautifully crafted set of songs". Spin magazine gave it eight out of ten, while Uncut magazine gave it a perfect five out of five, calling it an "inarticulate speech of the heart". Pneumonia peaked at #158 on the Billboard 200 and at #38 on the Heatseekers Albums chart. Prior to its release, Iha performed a handful of shows with Whiskeytown in 1998.
