{{Social media/Instagram|The Siamese Dream artwork was initially going to be created by an outsider artist, but after a series of disagreements with the label, Corgan was forced to step in at the last minute. The album booklet, composed of old photos of strangers and Corgan's family members with lyrics handwritten on top, was assembled by him and his wife the day after their wedding. Corgan was not satisfied with the results. The first pressing of the CD contained a 20-page booklet with a separate page devoted to each song's lyrics and accompanying photograph; later pressings contained a four-panel fold-out liner with thumbnails of each picture. In 1999, Virgin Records reissued the album with the original 20-page booklet.
{{Social media/Instagram|On July 30, 1998, The Smashing Pumpkins appeared on the Late Show with David Letterman in New York City to promote the upcoming single release of "Perfect". It was their first appearance on a David Letterman show. While the bulk of the show was taped inside the 457-capacity Ed Sullivan Theater as usual, the band played on a stage erected in the middle of 53rd Street, in front of the Hello Deli. Radio contest winners and Late Show audience members stood in front of the stage, while the general public were restricted to an area about 50 feet back from the stage. The band played five additional songs for those in attendance, although the show was ultimately shut down early by police due to crowd overflow.

Shortly after The Smashing Pumpkins reformed in 2007, Corgan posted a message to their blog saying that they were "looking for girls from Siamese Dream album cover… As you all know they were quite young when the photo was taken. They are not conjoined anymore, as far as we know." In 2011, there was brief confusion, or perhaps in jest, that then-bassist Nicole Fiorentino was one of the girls. This was later confirmed as false and both girls were located in 2008. Ten years later, in promotion of the Shiny and Oh So Bright tour, the two girls reunited for a photo op (pictured) and video op, using a flame thrower to ignite the classic SP heart logo. Corgan thanked them on Instagram, writing "we adore you [Ali and LySandra], and having you be a part of today's launch brings tears to my eyes. For life goes fast, and I can still see you in my mind's eye wearing crisp white dresses in a stranger's backyard, looking like little Mother Mary's, smiling and laughing into the sun."
David Letterman told the crowd before the performance, "We couldn’t think of a better way, a better reason for tying up traffic on 53rd Street than for a performance from this great rock and roll band, Smashing Pumpkins."

Corgan's father, William Corgan Sr, is credited as an illustrator for the 2011 reissue of Siamese Dream.
